About the Role

We are seeking a dedicated Public Health Nurse to join our team at Sheffield Children's NHS Foundation Trust. As a Community Health Specialist, you will work closely with a multidisciplinary team to support children, young people, and families in achieving their best outcomes.

Key Responsibilities

  • Deliver the Healthy Child Programme, ensuring comprehensive care and support for children and families.
  • Conduct thorough assessments of health needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with healthcare professionals, parents, and carers to provide coordinated care and support.
  • Refer to specialist services as needed, ensuring seamless transitions and continuity of care.
  • Contribute to the identification of public health needs, informing service development and improvement.
  • Participate in therapeutic programs, promoting healthy behaviors and outcomes.
  • Maintain accurate records and contribute to clinical audits, ensuring high-quality care and service delivery.

Requirements

Experience

  • Post-registration experience in healthcare, with a focus on community health and public health.
  • Experience working in multidisciplinary team settings, with a strong understanding of collaborative care.

Knowledge

  • Thorough understanding of the Healthy Child Programme, including its principles and implementation.
  • Knowledge of child development and wellbeing factors, including social determinants of health.

Qualifications

  • Registered Nurse with current NMC registration.
  • Additional qualifications or ongoing study in public health, community health, or a related field.
