About the Role
We are seeking a dedicated Lived Experience Link Worker to join our team. This exciting role will leverage your personal experiences to create meaningful connections and bridge the gap between mental health inpatient wards and community-based support services.
As a Lived Experience Link Worker, you will collaborate closely with frontline staff at MHM, Devon Partnership Trust professionals across Wonford House, and other organizations within the statutory and voluntary sectors. Your primary focus will be on fostering strong relationships with mental health inpatient wards and teams, facilitating seamless transitions to The Mooring Hub in Exeter and its comprehensive support network.
You will play a vital role in guiding patients and service users towards accessing essential community-based resources. This includes connecting them with relevant programs, services, and support groups tailored to their individual needs.
Key Responsibilities
- Build rapport and trust with individuals experiencing mental health challenges within inpatient settings.
- Provide guidance and information about community-based resources and support services available through The Mooring Hub and other organizations.
- Facilitate smooth transitions from inpatient care to community-based support, ensuring continuity of care and personalized assistance.
- Collaborate effectively with healthcare professionals, social workers, and other stakeholders involved in patient care.
- Advocate for the needs of individuals accessing mental health services, promoting their well-being and empowerment.
