Key Responsibilities

As a Community Nursing Specialist, you will:

  • Provide high-quality care within the community, assisting patients in their homes and promoting rehabilitation, self-management, and independence throughout their care journey.
  • Collaborate with a team of professionals to support evidence-based practices, fostering an environment of learning and development for staff.

The primary duties include managing diabetes, treating wounds (including leg ulcers), providing palliative care in close partnership with local hospices and general practitioners, and addressing bladder and bowel care needs. Additionally, you will work with a specific demographic of patients, including those who are severely frail, have dementia, are at the end of life, or have complex health conditions.

Job Responsibilities

Your responsibilities will include:

  • Delivering exceptional care in the community, focusing on rehabilitation, self-management, and independence for patients.

Qualifications

Essential:

  • Registered Nurse qualification (degree or diploma)
  • ENB 998 or equivalent, or willingness to pursue

Desirable:

  • Demonstrated clinical competencies across various areas

Experience

Essential:

  • Experience in a community healthcare setting

Skills and Knowledge

Essential:

  • Strong numeracy and literacy skills
  • Excellent communication and negotiation abilities
  • Competency in clinical skills such as wound management, catheterization, and palliative care
  • Ability to apply research-based practices
  • Familiarity with local policies affecting community health and social care
  • Understanding of the Community Nursing role
  • Promoting patient independence and choice
  • IT proficiency or willingness to develop skills
  • Knowledge of Clinical Governance and professional conduct
  • Ability to understand role boundaries
  • Experience as a practice supervisor
