As a Home Care Services Coordinator, your primary responsibilities will include:

  • Assessing the needs of Service Users through person-centred outcome planning.
  • Collaborating with the Service/Registered Manager and Service Users to effectively match staff to individual care requirements.
  • Maintaining comprehensive, person-centred support plans, risk assessments, and all relevant documentation for Service Users.
  • Coordinating shift coverage for Home Care Support Workers, which may occasionally require you to provide direct support.
  • Participating in an out-of-hours rota as needed.
  • Ensuring timely reporting of any changes to Service Users' schedules and arranging for replacement staff as necessary.
  • Updating support plans and Service Users' files, along with other administrative duties as required.
  • Delivering high-quality, person-centred personal care to Service Users, including intimate procedures when necessary.
  • Attending relevant meetings as directed by the Service/Registered Manager.
  • Participating in staff induction and training as required.
  • Ensuring all Service Users and staff files are maintained in accordance with company policies.

It is essential to engage in all necessary training to meet the standards set by Health and Social Care Standards and SSSC Codes of Practice, while also maintaining your SSSC registration.

A qualification such as S/NVQ level 3 in Adult Health and Social Care or a commitment to work towards this qualification is required.
