About the Role

Mid Norfolk Primary Care Network (PCN) is seeking a dedicated care co-ordinator to enhance our support for multiple GP practices and associated care facilities in the Dereham region. This role offers flexibility, with options for both full-time and part-time engagement.

The care co-ordinator will collaborate closely with GPs and practice teams within the PCN, playing a crucial role in ensuring that all patients receive optimal care and services.

Main Responsibilities
  • Serve as a key contact point to guarantee that necessary support is accessible.
  • Organize and prioritize tasks independently while contributing effectively as a team member.
  • Facilitate referrals to appropriate service providers.
  • Coordinate meetings, ensuring all relevant clinical and non-clinical personnel are present and prepared.
  • Encourage and promote collaboration between GP and community teams.
  • Handle sensitive information regarding complex issues with utmost confidentiality.
  • Draft comprehensive and clear minutes from meetings, distributing them to all relevant stakeholders with action items highlighted.
  • Provide guidance and support to patients and their families/carers as needed.
  • Maintain accurate electronic records of patient care and planned services, including hospital admissions and discharges.
  • Direct service users and carers to suitable services.
  • Participate in quality audits.
About the Candidate

The ideal candidate will possess strong skills in person-centered planning and have a solid understanding of local resources for support. We welcome applications from individuals who meet the following criteria:

Essential Qualifications
  • Proficient in Microsoft Office Suite.
  • Experience managing sensitive and confidential information.
  • Demonstrated administrative capabilities.
  • Knowledge of Clinical Governance principles.
Desirable Qualifications
  • Experience in a healthcare environment.
  • Familiarity with office procedures in an administrative capacity.
  • Understanding of database management.
  • Experience working within multidisciplinary teams.
  • Awareness of relevant health and social care legislation and initiatives.
