As a Senior Clinical Assessor, you will play a critical role in ensuring that individuals with mental health conditions and acquired brain injuries receive the necessary care and support to live fulfilling lives in their communities.
Key Responsibilities- Conduct thorough assessments of individuals' health and social care needs, including those with complex neurological conditions and acquired brain injuries.
- Develop and implement personalized care plans that promote independence, dignity, and well-being.
- Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and family members, to ensure seamless care coordination.
- Provide expert advice and guidance on mental health and acquired brain injury care, including the application of the Mental Capacity Act and relevant legislation.
- Contribute to the development and implementation of policies and procedures that promote best practice in mental health and acquired brain injury care.
- Registered Health or Social Care Professional with current registration with a professional body (NMC, HCPC, or SWE).
- Experience in assessment of health and social care needs, care planning, and coordination.
- Knowledge and experience working with individuals with mental health conditions and acquired brain injuries.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to analyze and interpret complex information, make informed decisions, and recommend appropriate courses of action.
