Role Responsibilities

Your Key Duties

The Field Care Supervisor plays a crucial role in ensuring the delivery of top-quality care support services. This includes conducting assessments and reviews of individual services, as well as providing practical supervision, guidance, and support to care workers.

Collaborating with Care Coordinators, you will assist in planning effective care worker schedules, considering the specific needs of clients and the unique skills of each worker. Regular reviews of individual services will be conducted, including ongoing needs and risk assessments. You will also participate in multi-disciplinary reviews of service users' requirements and mentor new care workers in the field to ensure their competence. On-site observation and evaluation of care worker performance will be part of your routine supervisory duties.

While the majority of your time will be spent in the community fulfilling your primary responsibilities, the role will also involve some office-based administrative tasks.

Qualifications

Essential Requirements

Experience in conducting initial risk and needs assessments for individual service users is necessary, whether in their homes or in other settings such as hospitals or residential care facilities. You will work collaboratively with service users, their families, caregivers, advocates, healthcare professionals, and others to ensure the seamless delivery of comprehensive care support.
