Care Coordinator

Job Summary
Magdalen Medical Practice is seeking a skilled Care Coordinator to join their team on a full or part-time basis. The successful candidate will provide extra time, capacity, and expertise to support the practice team in delivering high-quality care to patients.
Main Duties of the Job
The ideal candidate will have experience of working within healthcare, the voluntary or community sector, to support vulnerable groups. They will be familiar with local resources and services, including how to access them, and motivated by helping people with care and empathy. The successful candidate will be able to deliver person-centred support in a non-judgmental way and have experience of completing holistic person-centred care planning assessments.

Magdalen Medical Practice has a rich history dating back to the First World War. The practice has maintained its commitment to family medicine while embracing modern developments. The surgery is spacious, comfortable, and modern, with easy access via automatic main entrance doors and facilities for the disabled.
Job Responsibilities
The successful candidate will be responsible for providing person-centred support to patients, completing care planning assessments, and working with the practice team to deliver high-quality care.
