Job Title: Care Coordinator

Job Summary:

Magdalen Medical Practice is seeking a skilled Care Coordinator to join their team on a full or part-time basis. The successful candidate will provide extra time, capacity, and expertise to support the practice team in delivering high-quality care to patients.

Main Responsibilities:

  • Provide person-centred support to vulnerable groups, including those with complex needs.
  • Develop and implement holistic care plans, working closely with patients, families, and healthcare professionals.
  • Collaborate with local resources and services to ensure seamless access to support and care.
  • Deliver high-quality, empathetic care, prioritizing patient needs and preferences.
  • Contribute to the development of the practice team, sharing expertise and best practices.

Magdalen Medical Practice has a rich history, dating back to the First World War. Our modern surgery is spacious, comfortable, and accessible, with a strong focus on patient-centered care. Our catchment area includes Catton, Sprowston, Spixworth, and Heartsease.

Person Specification:

  • Experience of working within healthcare, the voluntary, or community sector, to support vulnerable groups.
  • Familiarity with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Able to deliver person-centred support in a non-judgmental way.
  • Experience of completing holistic person-centred care planning assessments.
  • Strong listening and communication skills.
