Care Coordinator

Job Summary
Magdalen Medical Practice is seeking a skilled Care Coordinator to join their team on a full or part-time basis. The post-holder will provide extra time, capacity, and expertise to our practice team to help us improve the care provided to our patients.
Main Duties of the Job
The ideal applicant will have experience of working within healthcare, the voluntary or community sector, to support vulnerable groups. They will be familiar with local resources and services, including how to access them. The successful candidate will be motivated by helping people, with care and empathy, and able to deliver person-centred support in a non-judgmental way.
About Us
Magdalen Medical Practice has a rich history, dating back to just after the First World War. The surgery is spacious, comfortable, and modern, with a car-park, easy access via automatic main entrance doors, hearing loop, and facilities for the disabled. Our catchment area includes Catton, Sprowston, Spixworth, and Heartsease.
Job Responsibilities
The successful candidate will be responsible for providing person-centred support to vulnerable groups, working closely with the practice team to improve the care provided to our patients. They will be expected to have strong listening and communication skills, and be able to deliver holistic person-centred care planning assessments.
