About the Role

We are seeking a highly skilled and compassionate Clinical Practitioner to join our Adult Community Learning Disability team. As a key member of our multidisciplinary team, you will work closely with service users, their families, and other healthcare professionals to provide high-quality, individualized care.

Key Responsibilities
  • Manage case work under the direction of a care coordinator/pathway clinical lead and provide ongoing assessment and recovery-focused care planning for service users with low-risk needs.
  • Deliver evidence-based clinical and psychologically based interventions, with appropriate training and supervision, to service users with complex and substantial needs.
  • Work collaboratively with the multidisciplinary team to ensure high-quality, individualized care that promotes recovery and wellbeing and maximizes independence.
  • Provide compassionate care that is based on empathy, kindness, respect, and dignity.
Requirements
  • Professional qualification and current professional registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
Desirable Qualifications and Experience
  • Experience of working within a community team.
  • Non-Medical Prescribing.
  • Additional training/qualifications in positive behaviour support.
