Job Title: CPN - Older Adults

We are seeking a skilled and compassionate Community Psychiatric Nurse (CPN) to join our team in South Buckinghamshire. As a CPN, you will play a vital role in delivering high-quality mental health services to older adults.

About the Role:

This is a challenging and rewarding opportunity for a dedicated and experienced CPN to work with our client's Older Adults Service. You will be responsible for participating in multi-disciplinary assessments, formulating and implementing care plans, and providing treatment as necessary.

Responsibilities:
  • Participate in multi-disciplinary assessments of patients' health conditions.
  • Formulate and implement care plans.
  • Provide treatment, as necessary.
  • Use evidence-based practice and specialist skills to assess, plan, implement, and evaluate interventions.
  • Collaborate with other healthcare professionals to ensure optimal patient care.
  • Maintain accurate and up-to-date records in line with professional standards.
  • Stay current with the latest mental health advancements and best practices.
Requirements and Qualifications:

To be successful in this role, you will need:

  • Current and valid registration with the NMC.
  • Proven experience within a community recovery role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
