Our team operates within a broader integrated care framework, focusing on delivering community-based rehabilitation services to individuals with intermediate care requirements. This encompasses intensive rehabilitation efforts and strategies to prevent hospital admissions. We collaborate closely with our colleagues in various urgent care services.

You will engage with a multidisciplinary team, including Occupational Therapists, Nurses, Consultants, and GPs, while receiving support from Assistant Practitioners and Rehabilitation Support Workers.

Your direct supervisor will be one of the senior physiotherapists in the team, ensuring that you are never working in isolation.

Key Responsibilities

The primary responsibilities for these Band 6 positions include:

  • Delivering effective community rehabilitation tailored to individual needs.
  • Collaborating closely with colleagues from various sectors, including local authorities and community organizations.
  • Providing guidance and training to junior staff and support workers on community rehabilitation practices.

Candidate Profile

Experience

Essential

  • Proven experience in community services, rehabilitation, neurology, or managing long-term conditions.
  • Experience working within a Physiotherapy team and the wider multidisciplinary team.
  • Skills in supervising and delegating tasks to junior therapists and support staff.
  • Contribution to the clinical education of less experienced colleagues or students.
  • Experience within the NHS is required.
  • A valid driver's license is necessary.

Desirable

  • Participation in audit and research initiatives.

Qualifications

Essential

  • Degree or Diploma in Physiotherapy.
  • Registration with the HCPC.
