Overview:
We are seeking compassionate and dedicated Care Workers to join our rehabilitation team. As a Rehabilitation Care Worker, you will provide essential support and assistance to individuals in their own homes on discharge from hospital with the aim of promoting independence and re-enablement. This is a rewarding opportunity to make a positive impact on the lives of others.

**Responsibilities**:

- Following personalised care plans to maximise independce
- Assist individuals with daily activities such as personal hygiene, dressing, and meal preparation
- Administer medication as directed by healthcare professionals
- Provide emotional support and companionship to individuals in your care
- Assist with light housekeeping tasks, including cleaning and organizing living spaces
- Help individuals with mobility issues, using proper lifting and transferring techniques
- Communicate effectively with individuals, their families, and healthcare professionals
- Utilize IT systems to accurately document care provided

Qualifications:

- Fluent in English, both written and verbal
- Previous experience in a care home or home care setting is preferred
- Basic IT skills for documentation purposes
- Ability to follow care plans effectively
- Excellent communication skills to interact with individuals, families, and healthcare professionals
- Valid driver's license and access to own vehicle.
