About Us:
Sandwell Metropolitan Borough Council Adult Social Care is currently seeking dedicated Social Workers to join our Borough-wide Hospital & Intermediate Care Team. This is an exciting opportunity to be part of a dynamic team operating a 7-day service including Bank Holidays.

**Job Description**:
As a Social Worker within our Hospital & Intermediate Care Team, you will play a crucial role in facilitating the timely discharge of individuals from hospitals into intermediate care settings. Working collaboratively with NHS partners and the ICB, you will demonstrate a strong customer focus and a commitment to promoting independence, choice, and control.

Key Responsibilities:
Utilize a strengths and asset-based approach in practice to empower individuals and promote their independence.

Undertake safeguarding investigations, mental capacity assessments, and best interest decisions.

Conduct outcome-focused assessments and support planning to meet the care and support needs of vulnerable adults and their carers.

Work with a diverse group of individuals and partner agencies to achieve positive outcomes.

**Requirements**:
Qualified Social Worker with experience in adult social work, particularly with older people, individuals with physical disabilities and sensory impairments, and those with mental health issues.

Sound knowledge and experience of relevant legislation including the Care Act 2014, Safeguarding, Mental Capacity Act 2005, and Mental Health Act.

Ability to work under pressure, with attention to detail, and a can-do approach.

Excellent communication and interpersonal skills.
