Job Summary

We are seeking an experienced Social Worker to join our Learning Disabilities Assessment & Care Management Team. The successful candidate will have a strong background in working with individuals with learning disabilities and their carers.

Key Responsibilities
  • Conduct reviews of existing LD service user's current packages of care
  • Develop and implement support plans that promote community inclusion and best value
  • Participate in the duty rota and have experience of working on a duty system
  • Undertake care act assessments, reviewing packages of care and undertaking mental capacity assessments
Requirements and Qualifications
  • Current and valid registration with the Social Work England (SWE)
  • Proven experience within a Social Working role
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Commitment to providing high-quality patient care
  • Car driver to travel across multiple sites
