Key Responsibilities:
  • Carry out tasks as directed by senior staff, including physical health monitoring, investigation, and phlebotomy as necessary.
  • Provide delivery of high-quality care and support to patients and their carers, using a person-centred approach.
  • Contribute to the planning, implementation, and evaluation of therapeutic programmes of care for patients.
  • Report any changes in condition or general level of needs of service users.
  • Ensure high standards and accurate verbal/written communication about patient care with all team members.
  • Develop and maintain good relationships, communication, and teamwork with all disciplines.
  • Participate in the Care Programme Approach.
  • Ensure that all duties are carried out to the highest possible standards in accordance with local and Trust policies and procedures.
  • Comply with local and Trust policies and procedures, including attending mandatory training and refreshers.
  • Ensure all administrative duties are completed on a timely basis.
  • Ensure attendance and documentation of supervision, both managerial and professional.
  • Contribute to the induction of new staff and students.
  • Undertake other appropriate tasks delegated by senior staff.
  • Participate in team meetings.
Requirements:
  • Educated to GCE/GCSE (Maths and English) or equivalent.
  • Willingness to undertake care certificate and further training relevant to the role.
Desirable Criteria:
  • Phlebotomy training and ECG training.
  • Experience of providing care.
  • Experience of working in a multi-disciplinary team.
  • Experience of working with a multi-cultural community.
Experience:
  • Previous experience of working in the NHS.
  • Experience of working in a caring capacity.
  • Experience of being part of a team.
Knowledge and Skills:
  • Ability to communicate effectively both orally and in writing.
  • Ability to work as an active and supportive member of the team.
  • Ability to observe and monitor progress and report any concerns.
  • Ability to organise workload and time efficiently under supervision.
  • Must hold a full UK Driving Licence.
  • Ability to work within the boundaries of the job description.
Personal Qualities:
  • Recognises the limits of own authority within the role.
  • Highly motivated and resourceful, with a proactive approach.
  • Able to prioritise own work with the aid of supervision.
  • Emotionally and physically resilient to withstand the demands of the post.
