Exciting Opportunity for Mental Health Practitioners
CREST (community rehabilitation and enhanced support team) is expanding and seeking dedicated Mental Health Practitioners to join our developing Complex Emotional Needs (CEN) pathway. This role involves coordinating care for individuals transitioning from Level 2 into the community, offering evidence-based, trauma-informed interventions, and managing risk within a community setting.
The CREST approach focuses on providing innovative ways of working with individuals who have spent extended periods in hospital, aiming to improve outcomes for this service user group. We are committed to continuous improvement and adapting to the evolving needs of the population.
Main Duties- Coordinate care within the Complex Emotional Needs pathway
- Assist individuals in addressing barriers to discharge and assessing rehabilitation needs
- Collaborate effectively with the multidisciplinary team and engage in multi-agency partnerships
- Deliver therapeutic interventions and foster hope and optimism
