Job Summary

The Chelsea and Westminster Hospital NHS Foundation Trust is seeking a highly skilled and experienced Specialist Outreach Nurse to join our Hepatology service. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care, advancing the body of knowledge through education and research, and contributing to the development of best practices in Hepatology.

Key Responsibilities
  1. Referral and Advice: Receive referrals from the healthcare team and provide expert advice and guidance on Hepatology-related matters.
  2. Cross-Site Working: Work across both Chelsea and Westminster Hospital and West Middlesex Hospital, as well as outreach clinics, to provide comprehensive care to patients.
  3. Assessment and Supervision: Conduct holistic assessments, provide supervision and guidance to colleagues, and order, conduct, and interpret relevant tests and investigations.
  4. Collaborative Care: Work with the multi-professional team to contribute expert knowledge and skills to patient care and treatment, addressing risk factors and health promotion needs.
  5. Patient Empowerment: Build trust and rapport with patients, enabling empowerment in their healthcare and advocating for treatment decisions and choices.
  6. Care Planning: Plan and conduct appropriate follow-up, closure, or transition of care for patients, maintaining accurate and contemporaneous records.
