Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, offering them hope, kindness, and a brighter future.

About the Role

This is a unique opportunity to make a significant difference in the lives of individuals impacted by substance use and homelessness. You will be embedded within outreach services, offering healthcare and clinical interventions as appropriate to the homeless population. Your expertise and passion will drive the implementation of innovative clinical interventions, ensuring that every individual receives the care and support they deserve.

Responsibilities
  • Provide high-quality clinical services to individuals experiencing homelessness
  • Develop and implement innovative clinical interventions to address the unique needs of each service user
  • Collaborate with outreach teams to ensure seamless delivery of healthcare services
  • Contribute to the development of policies and procedures to improve healthcare outcomes for homeless individuals
Requirements

To be successful in this role, you will need:

  • Registered Nurse qualification and experience in a relevant field
  • Strong clinical skills and knowledge of healthcare interventions for homeless populations
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
What We Offer

We offer a competitive salary range of £43,814.50 - £48,183.48 (pro rata for part-time hours) and Outer London Weighting of £2,163.27 (pro rata for part-time hours). You will also have the opportunity to work with a passionate team dedicated to making a difference in the lives of our service users.
