Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, addressing their unique healthcare needs and promoting overall well-being.

Key Responsibilities:
  • Provide comprehensive healthcare services, including assessments, diagnoses, and treatment plans, to individuals experiencing homelessness.
  • Develop and implement innovative clinical interventions to address the complex healthcare needs of this population.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and referrals.
  • Conduct outreach and engagement activities to build trust and establish relationships with service users.
  • Participate in quality improvement initiatives to enhance service delivery and outcomes.
Requirements:
  • Registered Nurse with current registration and a valid NMC PIN.
  • Experience working with vulnerable populations, particularly those experiencing homelessness.
  • Strong clinical skills and knowledge of healthcare services for homeless individuals.
  • Excellent communication and interpersonal skills, with the ability to build rapport with service users.
  • Ability to work independently and as part of a team, with a flexible approach to working hours.
What We Offer:
  • A competitive salary range of £43,814.50 - £48,183.48 (pro rata for part-time hours).
  • Inner London Weighting of £4,072.06 (pro rata for part-time hours).
