Job Summary

We are seeking an experienced Community Nurse to join our team in a part-time Overnight Community Nurse position. This role involves providing unplanned, urgent nursing care to patients in their homes, managing complex health issues and collaborating with other disciplines as required.

Main Duties

As a Community Nurse, you will work closely with the Overnight DN Hub, focusing on preventing avoidable acute admission. You will be competent in a range of nursing skills, including palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, and medication administration.

About the Role

This is a 15-hour per week position, working three 10-hour shifts over a seven-day period. You will need to be a car driver with a valid driving license and have access to transport with appropriate business insurance to travel throughout the Trust.

Requirements

  • Relevant degree in nursing or equivalent qualification
  • Professional knowledge acquired through degree supplemented by diploma-level specialist training, experience, and short courses
  • Evidence of continuing professional development
  • Non-Medical Prescriber or prepared to undertake a course
  • Appropriately registered with the Professional body as per post requirements
  • Significant experience in the required setting
  • Demonstrate clinical and managerial leadership ability
  • Must be a car driver with a valid driving license or have access to transport with appropriate business insurance to travel throughout the Trust
  • Excellent communication skills
