Job Summary

We are seeking an experienced Community Nurse to join our team in an Overnight Community Nurse position within the DN Night Service. This part-time role is based in Somerset and involves working 10-hour shifts, including weekends and bank holidays. As a Community Nurse, you will provide unplanned, urgent nursing care to patients in their homes, managing complex health issues and collaborating with other disciplines as required.

Main Responsibilities
  • Provide high-quality nursing care to patients in their homes, including palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, and medication administration.
  • Work closely with the Overnight DN Hub to prevent avoidable acute admissions and make autonomous clinical decisions.
  • Create personalized care plans and manage a range of complex health issues.
  • Collaborate with other disciplines, including social care and community services, to provide comprehensive care to patients.

Requirements
  • Relevant degree in nursing or equivalent qualification.
  • Professional knowledge acquired through degree supplemented by diploma-level specialist training, experience, and short courses.
  • Evidence of continuing professional development.
  • Non-Medical Prescriber or prepared to undertake a course.
  • Appropriately registered with the Professional body as per post requirements.
  • Significant experience in the required setting.
  • Demonstrate clinical and managerial leadership ability.
  • Must be a car driver with a valid driving license or have access to transport with appropriate business insurance to travel throughout the Trust as appropriate for the role.
