Position Overview
We are seeking a dedicated and innovative Rehabilitation Therapy Specialist to become a vital member of our multidisciplinary team. This role is primarily focused on delivering exceptional therapy services within a community-based rehabilitation framework.
As part of our integrated rehabilitation services, you will be responsible for providing high-quality therapeutic interventions to adults who require support following their discharge from acute care settings. Our aim is to facilitate recovery and enhance the quality of life for individuals in the community.
The successful candidate will engage with patients presenting with a diverse range of conditions, including neurological, orthopedic, respiratory, and mobility challenges. Each day will present unique opportunities and challenges, ensuring a dynamic and fulfilling work environment.
We encourage a collaborative approach, fostering a culture of teamwork across various services and settings. Our community rehabilitation model is designed to enhance access to timely and person-centered therapy assessments and interventions for individuals transitioning from hospital care.
Key Responsibilities
- Deliver a high standard of rehabilitation therapy services.
- Conduct comprehensive assessments and develop tailored treatment plans for patients with complex needs.
- Supervise and guide support staff and allied health professionals.
- Collaborate effectively within the multidisciplinary team to ensure optimal patient outcomes.
- Operate independently while receiving guidance from senior therapists.

Qualifications and Experience
Essential Requirements:
