About the Role

We are seeking a highly motivated and enthusiastic Community Support Worker to join our Community Rehabilitation Team. The successful candidate will have experience of working with individuals with severe mental illness and a strong understanding of the recovery model.

Key Responsibilities
  • Provide support to an allocated group of service users, promoting their recovery and helping to maintain them in their community environment.
  • Assist registered practitioners in assessment, planning, implementing, and evaluating individual care plans.
  • Ensure service users engage effectively with the agreed goal plan and access appropriate services.
  • Positively promote independent living of service users within the community.
  • Report regularly to social inclusion workers and keyworkers.
Requirements
  • Level 2 qualification plus training and/or experience to Level 3.
  • Good standard of general education including good numeracy and literacy skills.
  • Evidence of recent further development/training.
  • Experience of working with individuals with mental health needs.
  • Understanding of mental health issues and how they impact on individuals.
  • Good written and verbal communication skills.
  • Ability to form positive therapeutic relationships with service users and carers.
  • A current driving license with business insurance and access to a car during the working day is essential.
