This role is part of NHS Dorset's Personal Health Commissioning Department, which fosters a supportive and compassionate environment for all employees. Our team members have access to a variety of health and wellness resources designed to promote overall well-being and enhance workplace performance.
Key Responsibilities
The Continuing Health Care framework encompasses several distinct work streams, including:
- Assessment Teams, which involve Fast Track assessments, Funded Nursing Care, and Triage.
- Ongoing and Review Teams that oversee the well-being of individuals and evaluate the quality and effectiveness of care arrangements funded by CHC, adjusting care as necessary.
- Appeals, Disputes, and Retrospective Assessment Work, which includes facilitating discussions with patients, families, or representatives to review assessment outcomes, leading meetings with local authority representatives to reach agreements, and conducting assessments for previously unassessed care periods.
As a mobile practitioner, you will have the opportunity to work across these various teams based on current needs, typically for a duration of 3-6 months or longer. This structure allows you to gain diverse experiences and engage with a range of professionals, including nursing, social work, and allied health professionals.

Our Mission
The Health and Care Bill establishes Integrated Care Systems (ICSs) on a statutory basis, empowering them to better integrate health and care services, enhance population health, and mitigate health disparities.

Role Responsibilities
The successful candidate will play a pivotal role in supporting the Personal Health Commissioning (PHC) service across the region. Operating within the framework of the National Guidelines for Continuing Healthcare, the individual will provide a competent and effective level of service, consistently focusing on client needs.
The role involves supporting various work streams within the Continuing Health Care Processes, with assignments to teams facing the highest demands. This may include working within Triage/Assessment Teams, Ongoing and Review Teams, or Appeals and Retrospective Assessment Teams.
As a Senior Care Coordinator, your responsibilities will be tailored to meet service demands, offering a diverse caseload.
The successful candidate will join a supportive and established team of professionals committed to delivering the most efficient and person-centered experience for individuals accessing this service.
Applicants should possess extensive experience in assessing and reviewing complex cases, along with a strong understanding of national frameworks.
Effective communication skills are essential, particularly in navigating emotionally challenging and contentious situations. The role requires the ability to convey complex, sensitive information and negotiate with senior stakeholders on difficult issues.
Given the nature of this position, candidates should demonstrate a reasonable level of physical, emotional, and mental resilience, including the ability to manage shifting priorities and competing agendas while dealing with emotionally charged circumstances.
A full UK driving license and access to a vehicle are required to fulfill the responsibilities of this role.
