Job Summary

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team as a Medicines Optimisation Specialist. The successful candidate will play a key role in leading our medicines optimisation efforts, working closely with our general practice team to improve patient care and outcomes.

Main Responsibilities
  • Lead the development and implementation of medicines optimisation strategies to improve patient care and reduce healthcare costs.
  • Work closely with our general practice team to identify areas for improvement and develop targeted interventions.
  • Conduct clinical assessments and provide expert advice on medicines management to patients and healthcare professionals.
  • Develop and maintain relationships with key stakeholders, including general practitioners, pharmacists, and other healthcare professionals.
  • Stay up-to-date with the latest evidence-based guidelines and best practices in medicines optimisation.
Requirements
  • Minimum of 2 years post-qualification experience in a primary care or hospital setting.
  • Good knowledge of NHS medicines optimisation policies and guidelines.
  • Experience of managing and developing a team.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work outside of core office hours.
