Job Title: Stroke Rehabilitation Therapy Associate Practitioner

We are seeking a highly motivated and experienced Therapy Associate Practitioner to join our Early Supported Discharge (ESD) service. The successful candidate will have a strong background in psychology, physiotherapy, occupational therapy, speech and language therapy, or healthcare assistance, and will be passionate about enhancing the lives of stroke survivors.

The ESD service is a key component of our Integrated Community Stroke Service (ICSS) model, which aims to deliver transformation and strategic changes in community stroke rehabilitation and life after stroke services. As a Therapy Associate Practitioner, you will work closely with a multidisciplinary team to provide high-quality care to patients with complex needs.

Key responsibilities will include:

  • Working within a multidisciplinary framework to provide key-working support for non-complex patients
  • Liaising with medical, nursing, allied health professionals, social care, acute, and voluntary sector staff to promote greater understanding of the service
  • Implementing, evaluating, and modifying interventions as directed
  • Being responsible for a delegated non-complex caseload, carrying out interventions, including goal setting, outcome measurement, risk assessment, and discharge planning

We are looking for an individual who has:

  • Relevant experience in a health and social care setting and a base level of knowledge in psychology, occupational therapy, physiotherapy, speech and language therapy, or healthcare assistance
  • Knowledge of patient/client conditions relevant to community stroke rehabilitation
  • Ability to work under own direction
  • Able to travel to meet the needs of the role
