Job Title: Stroke Rehabilitation Therapy Associate Practitioner

We are seeking a highly motivated and experienced Therapy Associate Practitioner to join our Early Supported Discharge (ESD) service. The successful candidate will have a strong background in psychology, physiotherapy, occupational therapy, speech & language therapy, or healthcare assistance, and will be passionate about enhancing the lives of stroke survivors.

The ideal candidate will have a solid understanding of patient/client conditions relevant to community stroke rehabilitation and will be able to work under their own direction. They will also be able to travel to meet the needs of the role.

Main Responsibilities:
  • Work within a multidisciplinary framework, undertaking a key-working role for non-complex patients as triaged by registered staff.
  • Liaise closely with medical, nursing, allied health professionals, social care, acute and voluntary sector staff to promote greater understanding of the service.
  • Implement, evaluate, and modify interventions as directed.
  • Be responsible for a delegated non-complex caseload following assessment by registered staff.
Requirements:
  • Completed Associate Practitioner Course (Foundation Degree) or relevant experience.
  • Car Driver and access to vehicle.
Person Specification:
  • Knowledge of relevant patient/client conditions.
  • Experience working with multidisciplinary teams.
