About the Role

We are seeking a highly skilled and experienced Senior Practitioner to join our team at Cheshire and Wirral Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for leading the development and delivery of complex care plans for patients with learning disabilities.

Key Responsibilities
  • Lead the development and delivery of care plans for patients with complex needs
  • Work collaboratively with the nursing team to provide high-quality care
  • Support the Ward Manager and Clinical Lead with supervisions and other tasks
  • Ensure that the service is based on an ethos of collaborative working between service users, carers, and stakeholders
Requirements
  • Post-registration experience of working with adults with learning disabilities
  • Experience of working within an assessment and treatment unit
  • Ability to work collaboratively as part of a multidisciplinary team
  • Strong communication and interpersonal skills
