Nurse Discharge Coordinator

We are seeking a skilled and experienced Registered General Health Nurse to join our team as a Discharge Coordinator. In this role, you will play a crucial part in ensuring the smooth and timely transfer of patients from our acute hospital setting to their community or home setting.
Key Responsibilities- Provide expert nursing care and support to patients and their families during the discharge process
- Work closely with ward teams to identify patients with complex needs and develop effective discharge plans
- Ensure that all necessary information is captured and recorded on the Frontier system
- Collaborate with community and social services to ensure seamless transitions of care
- Develop and maintain strong relationships with patients, families, and healthcare professionals to promote effective communication and coordination
- Current NMC registration
- Experience in acute hospital settings and discharge planning
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of clinical governance frameworks and evidence-based practice
