Position Overview

We are seeking a dedicated and enthusiastic Patient Care Coordinator to join our dynamic team on a fixed-term basis. This role is essential in supporting our healthcare delivery and enhancing patient experiences. The successful candidate will collaborate with a variety of healthcare professionals to ensure that patients receive comprehensive and coordinated care.

Key Responsibilities

  • Identify and assist individuals, particularly the elderly and those with chronic conditions, by providing navigation and coordination of appointments.
  • Engage with our multidisciplinary team to ensure seamless communication and support for patients and their families.
  • Utilize healthcare IT systems to conduct searches and maintain accurate patient records, ensuring timely follow-ups and care continuity.
  • Work closely with clinical staff on various projects aimed at improving patient outcomes and experiences.
  • Support the organization of community health initiatives and educational sessions for patients and caregivers.
  • Assist in the management of vaccination programs and other public health initiatives.

Qualifications and Skills
