About the Role

We are seeking a skilled and compassionate Healthcare Support Worker to join our GOSPORT Community Care Team. As a key member of our team, you will provide high-quality care to housebound patients with complex care needs, enabling them to receive the care they need in their own home.

Key Responsibilities
  • Participate in the delivery of patient care under the supervision of the Community Care Team.
  • Work under supervision following written care plans and care pathways devised by the Registered Nurse.
  • Follow prescribed care plans and treatment protocols.
  • Follow HPCT Policies and Procedures.
  • Delegated responsibility for Patient Support Teams admin.

Requirements
