About the Role

Southern Health NHS Foundation Trust is seeking a motivated and caring individual to join our GOSPORT Community Care Team as a Healthcare Support Worker. This part-time substantive position involves working weekdays and one weekend in four, providing care to housebound patients with complex care needs in their own homes.

As a Healthcare Support Worker, you will be responsible for participating in the delivery of patient care under the supervision of the Community Care Team. You will work under supervision following written care plans and care pathways devised by the Registered Nurse, and follow prescribed care plans and treatment protocols.

You will have delegated responsibility for Patient Support Teams admin and will be expected to report to the Registered Nurse any variances. This is an excellent opportunity for someone looking to join a community team or broaden their experiences, with excellent training offered around clinical skills and opportunities for further education and career development.

Person Specification

We are looking for someone with relevant experience in healthcare and a proactive and positive attitude to learning the skills required to fulfill the role. The ideal candidate will have an NVQ2 in Health care or equivalent, and a current driving license.
