About the Role

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team at Primary Integrated Community Services. As a Clinical Pharmacist, you will play a pivotal role in improving the quality of care and operational efficiencies in General Practice.

Main Responsibilities
  • Provide independent prescribing services to patients with single or multiple medical problems, including COPD and asthma.
  • Review patients' ongoing medication needs, monitoring requirements, and provide support to ensure optimal use of medicines.
  • Analyse and present medicines data to highlight issues and risks, supporting decision-making.
  • Ensure patients are referred to the appropriate healthcare professional for the right level of care within a suitable timeframe.
  • Manage repeat prescribing, including reviewing patient requests and implementing a practice repeat prescribing policy.
  • Identify and mitigate risks associated with medicines, including those related to patient safety and quality of care.
  • Collaborate with GPs, nurses, and other healthcare professionals to improve patient outcomes and operational efficiencies.
Key Requirements
  • Mandatory registration with the General Pharmaceutical Council.
  • Qualified Independent Prescriber.
  • Appreciation of primary care prescribing, rational prescribing, and strategies for improving prescribing.

  • Participate in the delivery of formal education programmes and ensure appropriate clinical supervision.
Research and Evaluation
  • Demonstrate ability to critically evaluate and review literature.
  • Identify gaps in the evidence base and generate evidence suitable for presentations.
  • Apply research evidence base into working place and demonstrate understanding of principles of research governance.
Health and Safety/Risk Management
  • Comply with the Practice's Health and Safety policies and follow agreed safe working procedures.
  • Comply with the Data Protection Act and the Access to Health Records Act.
