Key Responsibilities- Provide and maintain a high standard of nursing care to the practice population.
- Focus on delivering evidence-based practice for patients with long-term conditions and providing preventative health care to the practice population.
- Provide nursing care to practice patients, including primary care treatment, preventative care, screening, and education.
- Support efficient and effective service delivery to the practice population of approximately 10,792 patients.
- Take an active role in supporting the practice to maximize achievement against local and national targets in respect of the PMS contract, QOF, and Enhanced Services.
- Registered Nurse with sufficient demonstrable experience post qualification.
- GCSE grade A to C in English and Maths.
