Job Description

Job Title: Transformation Unit Community Nurse

Job Summary:

We are seeking a highly motivated and experienced Community Nurse to join our Transformation Unit team. As a Community Nurse, you will be responsible for providing high-quality patient care in a community setting, working closely with other healthcare professionals to achieve the best outcomes for patients.

Main Responsibilities:

  • Provide holistic care to patients in their own homes, GP premises, or other community settings.
  • Work as part of an integrated multidisciplinary team to deliver high-quality care and promote health and well-being.
  • Collaborate with other healthcare professionals, including GPs, Rapid Response teams, and Advanced Nurse Practitioners, to prevent hospital admissions and facilitate hospital discharges.
  • Assess and evaluate patient health needs, particularly in areas such as palliative care, wound management, diabetes, and other long-term conditions.

Requirements:

  • Registered Nurse Level 1 (Adult) qualification.
  • Mentor and assessor qualification or equivalent.
  • Evidence of continued professional development relevant to the role.
  • Experience of working in a multi-disciplinary team setting.
  • Experience in Community Nursing.

Personal Qualities:

  • Ability to work under pressure.
  • Effective time management.
  • Flexibility to work across various sites.

Working Conditions:

  • Independently mobile, requiring a car.
  • Willingness to work weekends and Bank holidays.
