Hospital Outreach Role

We are seeking a skilled and compassionate Drug and Alcohol Outreach Specialist to join our team in Cornwall. As a key member of our Hospital Outreach service, you will work from a trauma-informed perspective, providing support to individuals who are frequent attendees at the Royal Cornwall Hospital and struggling with substance use issues.

This is an exciting opportunity to work within a multi-disciplinary model of practice, collaborating with partners in safeguarding, housing, and homelessness services, as well as primary and secondary health care and mental health providers.

Key Responsibilities:

  • Provide outreach support to clients around Cornwall with drug and alcohol issues who are struggling to maintain their housing.
  • Work alongside the Adult Community Service and the Young People's Service to ensure an accessible treatment offer is available to those who find themselves rough sleeping or in insecure accommodation.
  • Develop and maintain relationships with local services and stakeholders to ensure a comprehensive support network for clients.
  • Contribute to the development and implementation of service plans and strategies to address substance use and homelessness.

Requirements:

  • A strong understanding of the problems faced by individuals finding themselves homeless or in insecure accommodation.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • A full UK driving licence and access to a vehicle, with occasional weekend work required.
