About the Role

We are seeking a dedicated and compassionate individual to join our team as a Drug and Alcohol Outreach Worker. As a key member of our outreach team, you will work with individuals struggling with substance misuse and homelessness, providing support and guidance to help them achieve their goals.

Key Responsibilities
  • Provide outreach support to individuals with substance misuse issues, working in a trauma-informed manner to build trust and rapport.
  • Collaborate with internal and external stakeholders, including healthcare providers, housing services, and social care agencies, to ensure a comprehensive support package for service users.
  • Develop and maintain relationships with service users, understanding their unique needs and circumstances, and working with them to create personalized support plans.
  • Conduct assessments and risk assessments to identify service users' needs and develop strategies to address these.
  • Work with service users to develop and implement care plans, ensuring they have access to the resources and support they need to achieve their goals.
  • Provide education and support to service users on substance misuse, harm reduction, and healthy living.
  • Participate in regular team meetings and contribute to the development of outreach strategies and initiatives.
Requirements
  • Experience working in the field of substance misuse, homelessness, or a related field.
  • Knowledge of trauma-informed practice and its application in outreach work.
  • Ability to work effectively with diverse populations, including those with complex needs and challenging behaviors.
  • Excellent communication and interpersonal skills, with the ability to build rapport with service users and stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
  • Full UK driving license and access to a vehicle.
