Hospital Outreach

Our Hospital Outreach role offers an exciting and challenging role that works from a trauma-informed perspective based in the community covering all of Cornwall. 

The service provides a rapid response to individuals who are frequent attendees at the Royal Cornwall Hospital in Truro and have issues related to substance use.

The team works within a multi-disciplinary model of practice with partners in safeguarding, housing and homelessness services, domestic abuse services, as well as primary and secondary health care and mental health providers.

Drug and Alcohol Outreach

This role will be working alongside other services providing outreach support to clients around Cornwall with drug and alcohol issues who are struggling to maintain their housing. You will work alongside the Adult Community Service and the Young People’s Service to ensure that an accessible treatment offer is available to those who find themselves rough sleeping or in insecure accommodation. 

You will be a resilient and committed individual who has an understanding of the problems faced by individuals finding themselves homeless or in insecure accommodation.

Floating Support Outreach 

As a Floating Support Outreach Worker, you will be trained in all specialisms mentioned above and will provide support to the teams, depending upon where the need is greatest.

Due to the outreach basis of these role, a full UK driving licence and access to a vehicle is required. The post holder will be required to work 37.5 hours per week with occasional weekend work.

Required Skills

Evidence of continuous professional development. OCN/NVQ level 3 or equivalent or willing to achieve this within 12 months. Demonstrable experience working in the fields of outreach, housing/homelessness health, social care, criminal justice, domestic abuse, or another specialist setting Successful track record of working with substance misuse clients Resilient and the ability to prioritise and work under pressure Knowledge and understanding of health and safety, including assessing high-risk situations for service users Knowledge and understanding of homelessness, assessment, care planning and key working Ability to build effective relationships with a range of internal and external stakeholders Ability to work effectively as a team player. Full UK Driving Licence and own transport (the role will involve travelling to a variety of centres across Cornwall) Ability to work flexible and unsocial hours as required
