Matron Hospital at Home Lead

We are seeking an experienced and skilled professional to take on the role of Matron Hospital at Home Lead at Essex Partnership University NHS Foundation Trust. As a key member of our leadership team, you will be responsible for the efficient and effective day-to-day running of the Hospital at Home (HatH) and Urgent Community Response Service.
Key Responsibilities- Manage the operational management of the HatH service, ensuring all patients receive safe and compassionate care and experience a great service.
- Lead a team of multi-disciplinary clinical staff, ensuring resources are used to maximise the benefit to all patients.
- Develop specialist skills within the team to help reduce the demand on acute sector care.
- Monitor and audit clinical care, sharing learning across the organisation.
- Lead daily MDTs and contribute to case conferences, ensuring a holistic assessment of patients as required.
- Set outcome-based targets for the service and manage performance against these.
- Respond to informal and formal complaints, making changes to services, systems, policies, practice, and communication as appropriate.
- Communicate complex information to staff regarding service developments.
- Network with and establish effective partnership working with key personnel.

We are looking for a candidate with the following qualifications and experience:
- Registered Nurse or AHP Qualification
- Registered with a clinical registration body
- Educated to degree level or relevant equivalent experience
- Significant experience of managing a large multi-disciplinary team of health and ideally social care staff
- Senior level clinical knowledge in community unplanned/urgent care
- Possess extensive working knowledge of Primary and Community services
- Management of change
