Primary Care Network Senior Clinical Pharmacist

Join Greenwich Health Ltd as a Primary Care Network Senior Clinical Pharmacist and play a pivotal role in improving the quality of care and operational efficiencies in our primary care networks.
About the RoleWe are seeking an experienced and skilled Clinical Pharmacist to join our team in South East London. As a Primary Care Network Senior Clinical Pharmacist, you will work closely with our GP Federation, PCNs, and local practices to provide clinical leadership on medicines optimisation and quality improvement.
Key Responsibilities- Proactively manage patients with co-morbidities and provide clinical medication reviews to ensure safe and effective medication use.
- Provide patient-facing clinics for those with questions, queries, and concerns about their medicines.
- Manage medicines at discharge from hospital, including reconciling medicines and identifying and rectifying unexplained changes.
- Work in partnership with hospital colleagues to proactively manage patients at high risk of medicine-related problems before they are discharged.
- Manage own case load and run long-term condition clinics, prescribing as an independent prescriber if suitably qualified.
- Provide telephone medicines support and answer medicine-related enquiries from GPs, other practice staff, and patients.
- Devise and implement practice searches to identify cohorts of patients most likely to be at risk of unplanned admissions and readmissions from medicines.
- Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time.
- Produce and implement a practice repeat prescribing policy and manage the repeat prescribing reauthorisation process.
- Masters degree in Pharmacy.
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
- Minimum 5 years post-qualification experience.
- In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- An appreciation of the nature of GPs and general practices.
- Registration with the General Pharmaceutical Council.
- Membership of the Royal Pharmaceutical Society (desirable).
