- Work closely with general practice teams to ensure seamless care transitions and improve patient outcomes.
- Develop and implement clinical protocols and guidelines to improve patient care and safety.
- Conduct medication reviews, provide education, and support patients to take their medications effectively.
- Collaborate with community pharmacists, hospital pharmacists, and other healthcare professionals to improve patient experience and manage incidents.
- Support the development and implementation of new services that have medicinal components, such as treatment pathways and patient information leaflets.
- Monitor and analyse medicines data to highlight issues and risks, and contribute to decision-making.
- Implement changes to medicines that result from MHRA alerts, product withdrawal, and other local and national guidance.
- Assist in the development and maintenance of a practice formulary and ensure compliance with local medicines management requirements.
- Registered pharmacist with a strong clinical background and experience in primary care.
- Excellent communication and leadership skills, with the ability to work effectively with multidisciplinary teams.
- Knowledge of medicines management, patient safety, and quality improvement principles.
- Ability to work independently and as part of a team, with a strong focus on patient-centred care.
- Basic knowledge of Quality Improvement (QI) methodology and ability to apply it in practice.
