Job Description

Join our team as a Clinical Pharmacist in Primary Care Network, where you will play a key role in developing and delivering high-quality pharmacy services to patients in our community.

Key Responsibilities
  • Provide clinical leadership and expertise in medicines optimisation, patient safety, and quality improvement.
  • Work closely with general practice teams to ensure seamless care transitions and improve patient outcomes.
  • Develop and implement clinical protocols and guidelines to improve patient care and safety.
  • Conduct medication reviews, provide education, and support patients to take their medications effectively.
  • Collaborate with community pharmacists, hospital pharmacists, and other healthcare professionals to improve patient experience and manage incidents.
  • Support the development and implementation of new services that have medicinal components, such as treatment pathways and patient information leaflets.
  • Monitor and analyse medicines data to highlight issues and risks, and contribute to decision-making.
  • Implement changes to medicines that result from MHRA alerts, product withdrawal, and other local and national guidance.
  • Assist in the development and maintenance of a practice formulary and ensure compliance with local medicines management requirements.
Requirements
  • Registered pharmacist with a strong clinical background and experience in primary care.
  • Excellent communication and leadership skills, with the ability to work effectively with multidisciplinary teams.
  • Knowledge of medicines management, patient safety, and quality improvement principles.
  • Ability to work independently and as part of a team, with a strong focus on patient-centred care.
  • Basic knowledge of Quality Improvement (QI) methodology and ability to apply it in practice.
