As a key member of our Older People's Mental Health Liaison Service, you will play a vital role in providing high-quality care and support to older adults with mental health needs.
Key Responsibilities- Act as a resource clinician, providing support and clinical advice to patients, families, and healthcare professionals.
- Ensure the implementation of all Trust policies and procedures, promoting clinical excellence and best practice.
- Develop and maintain effective relationships with multidisciplinary teams, including consultants, nurses, and other healthcare professionals.
- Conduct comprehensive assessments, identify needs, and develop personalized care plans to support older adults with mental health needs.
- Collaborate with community mental health teams, care homes, and other stakeholders to ensure seamless care transitions and coordination.
- Provide education and training to healthcare professionals, promoting knowledge and skills in mental health care for older adults.
- Contribute to the development and implementation of service policies, procedures, and guidelines, ensuring they are evidence-based and effective.
- Participate in quality improvement initiatives, audits, and research projects to enhance service delivery and patient outcomes.
We are looking for a highly motivated and experienced Mental Health Liaison Practitioner with:
- Registration as a Mental Health Nurse, Occupational Therapist, or Social Worker.
- At least 2 years of experience in a relevant setting, with a strong understanding of mental health care for older adults.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Strong assessment and planning skills, with the ability to develop and implement effective care plans.
- Knowledge of relevant policies, procedures, and guidelines, with the ability to apply them in practice.
