Job Summary

The Bradford District Care NHS Foundation Trust is seeking a skilled Mental Health Liaison Practitioner to join our team. As a key member of our Older People's Mental Health Liaison Service, you will play a vital role in supporting patients in acute Trust ward areas and collaborating with multidisciplinary teams to provide high-quality care.

Key Responsibilities
  • Act as a resource clinician, providing support and clinical advice to patients and healthcare professionals.
  • Ensure the implementation of Trust policies and procedures, promoting clinical excellence and best practices.
  • Conduct assessments, make referrals, and coordinate care plans for older adults with mental health needs.
  • Collaborate with clients, carers, and healthcare professionals to promote participation in care planning and delivery.
  • Contribute to the planning, monitoring, and control of resources, ensuring agreed standards of care are achieved.
  • Apply the Mental Health Act (1983) and maintain knowledge of Directorate policies and procedures.
  • Ensure equality of opportunity and fair access for all service users and carers.
  • Take a lead role in addressing clinical governance, research, audit, and good practice within the service.
  • Develop an effective communication network through liaison and collaboration with other disciplines, services, and agencies.
