Trust: Bridgewater Community Healthcare
Shift Patterns: 6 or 12 hour shifts Days/Nights
You will be working as part of a dedicated integrated health and social care team, providing rehabilitation for patients in a step-down and step-up setting. To be successful in this role, you will need to have a solid foundation of basic nursing skills and be able to support the needs of patients with a range of health conditions.
Key Responsibilities:
- Perform comprehensive assessments of patient needs and develop individualized care plans
- Work collaboratively with the integrated health sister/charge nurse and wider healthcare professionals to ensure effective service delivery
- Provide high-quality nursing care to patients admitted to the facility, including administering medications, performing procedures, and providing emotional support
- Develop and maintain effective relationships with patients, families, and healthcare teams
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements:
- A minimum of 6 months paid experience in a ward-based nursing environment within the UK, preferably within an Acute or Community NHS Trust or Private Hospital
- Effective interpersonal and relationship-building skills
- Ability to work flexibly, including evenings, nights, and weekends
- Commitment to delivering high-quality patient care and contributing to a positive and supportive team environment
