Job Summary

Rayleigh and District Primary Care Network is seeking a dedicated and experienced advanced nurse practitioner to join our team on a full-time basis. As a key member of our multidisciplinary team, you will play a pivotal role in delivering exceptional healthcare services to our community.

Main Responsibilities

Based in our Rayleigh practice, you will be responsible for conducting comprehensive patient assessments, diagnosing and managing a wide range of conditions, and providing expert advice and education to patients and their families. You will also collaborate with the multidisciplinary team to develop and implement care plans that meet the individual needs of patients.

Person Specification

Qualifications

Essential

  • Master's in advanced clinical practice or an equivalent MSc or NHS England Centre for Advancing Practice Digital Badge (or working towards it).
  • Active NMC registration and licence to practice.
  • 4 years experience as an advanced nurse practitioner, working at a masters level that encompasses the four pillars of clinical practice, leadership and management, education, and research.
  • Possess a prescribing qualification.
