About the Role

We are seeking a highly skilled and compassionate Community Psychiatric Nurse to join our Crisis Resolution Home Treatment Team. As a key member of our team, you will play a vital role in delivering high-quality care to individuals experiencing mental health crises.

Key Responsibilities
  • Work collaboratively with multidisciplinary teams to provide comprehensive care to patients and their families.
  • Conduct thorough assessments and develop individualized care plans to address the unique needs of each patient.
  • Provide crisis intervention and stabilization services to individuals in their homes, reducing the need for hospitalization.
  • Collaborate with colleagues to develop and implement risk management plans, ensuring the safety and well-being of patients and staff.
  • Lead joint reviews and develop transfer of care plans for patients with complex needs.
  • Ensure patients are actively involved in their care planning and decision-making processes.
Requirements
  • Registration with the Nursing and Midwifery Council (NMC).
  • Experience working in mental health services, with a focus on crisis intervention and home treatment.
  • Ability to work a 24-hour shift pattern, including day shifts, night shifts, and weekend shifts.
  • Full driver's license and ability to drive to different sites.
