Responsibilities

* Develop and deliver training programs to patients, carers, and healthcare professionals to support safe discharges from the hospital and ongoing care at home.
* Conduct regular patient reviews and provide support with questions, issues, and troubleshooting requests.
* Collaborate with the healthcare team to ensure effective patient care and support.
Requirements
------------
* Registered Nurse (RGN, RSCN, or RNLD) with at least 12 months of experience.
* Living in Hampshire and the surrounding area.
* Full UK driving license.
