Job Overview

The role involves delivering a thorough and effective pharmaceutical service, adhering to established standards of quality and professional practice to fulfill the needs of users.

Responsibilities include providing high-quality pharmaceutical care to patients and their caregivers.

Promoting evidence-based and cost-effective medication use, effective risk management, clinical governance, and compliance with policies, procedures, and guidelines related to medication use is essential.

Main Duties of the Job

1. Deliver a comprehensive pharmaceutical service, aiming for expert-level knowledge.

2. Enhance managerial skills and expertise by working towards objectives agreed upon with the Principal Pharmacist and Deputy Chief Pharmacist.

3. Collaborate closely with the Principal Pharmacist to contribute to the maintenance and development of evidence-based treatment guidelines and protocols as required.

4. Work in conjunction with pharmacy leads to ensure that practice changes meet required standards and are implemented Trust-wide.

5. Act as a deputy for the Principal Pharmacist as necessary.

6. Monitor the pharmacy service, working with pharmacy leads to initiate improvements or changes as needed.

7. Appropriately escalate issues that may affect the delivery of routine pharmacy services.

8. Report and investigate errors, complaints, and incidents in line with departmental procedures.

9. Facilitate the sharing of learning from errors, complaints, and incidents.

10. Conduct risk assessments and manage and report risks as required.

11. Attend and participate in departmental Clinical Governance meetings as appropriate.

12. Produce rotas for dispensary and ward cover.

13. Undertake dispensing and checking of prescriptions in accordance with departmental policies.

14. Participate in the clinical pharmacy service to monitor prescriptions.

15. Engage in daily multidisciplinary team ward rounds, providing expert pharmaceutical advice.

16. Conduct medication reconciliation for each patient upon admission to ensure prescribed medications are appropriate.

17. Screen medication drug charts and prescriptions to ensure the safe and legal standard of supply.

18. Review and validate medicines reconciliations completed by Clinical Pharmacy Technicians in a timely manner.

19. Collaborate with clinical pharmacy technicians and assistants to ensure a seamless and efficient patient-centered service.

Qualifications / Education

Essential Criteria

  • Registered Pharmacist with the General Pharmaceutical Council
  • Pharmacy degree – MPharm Pre-registration training
  • Diploma in Clinical Pharmacy or equivalent

Desirable Criteria

  • MSc in Clinical Pharmacy Practice or equivalent. Management training

Experience

  • Post-registration UK hospital experience
  • Demonstrated evidence of excellent clinical knowledge and skills
